• Identification for: yourself/dependent/partner/other e.g. photo ID, birth certificate, social security or Medical card, etc. 

• Proof of address e.g. bill for electricity, water, gas, etc., 

• Proof of Income (for all adults in the household) e.g. direct deposit, check stubs, government letter, no income self-certification, cash income, etc. 

Once submitted you will receive a follow up call or email within five business days.
